Provide ongoing case management children and families with qualifying disabilities and Medical Assistance in Children’s Long Term Support program.  Assist with resources and services to children enrolled in program.    

QUALIFICATIONS:  Bachelor’s degree from accredited school in Social Work, human services or related field; one year of related experience with individuals with disabilities or mental illness; insurance acceptable driver’s license; Qualify under WI Caregivers Law, WI Act 27. 

• 40 Hours/week. 
• $26.18 - $31.76/hour.
